At-home Care Tracking for a Health and Wellness Organisation

A simplified health journey
The team partnered with Apple Healthcare to pilot a mobile first product ecosystem for both patients and care providers. The ecosystem aimed at connecting care teams members and patients to actively monitor their health and facilitate informed conversations between them to improve their post discharge experience.
The aim was to deliver a platform with a scalable, extensible architecture to deliver immediate value in cardiovascular disease while laying the foundation for scaling to other diseases.
The design team worked with a group of trained care professionals to conduct discovery. Focus group sessions were conducted with patients and care givers to arrive at the core functionalities to be released in the pilot.
A native iOS app was created for the patients with chronic progressive condition and an iPad app for care team professionals.

Congestive Heart Failure (CHF)
Heart failure is the leading cause of hospitalisation and readmission among older adults. Readmission rates for CHF are approximately 20-25% at 30 days and over 50% at 6 months.
Addressing CHF as the use case provides the opportunity to,
• Meaningfully improve the patient experience
• Create a differentiated experience for the care team
• Potentially reduce readmissions
Design Research
Multiple research workshops were conducted to gain insights into patient illness experience, their habits of technological engagement and key areas of focus for health care professionals. We conducted evaluative screen testing and the onboarding experience for patients on low-fidelity design mock ups.
